Instructions for filling out the form:
Preferred Method:
1. Download the form
2. Open using Adobe Acrobat (click the link to download the free software if you don't have it already)
3. Type all info directly into the form (official signatures will be signed at next in-person meeting)
4. SAVE!
5. Email completed form to [email protected]
OR
-Download the form, print out, fill out by hand and return to Pam Van Natter at next in-person meeting.
